Don’t Put The Cart Before The Horse

People often jump at anything that promises them fast relief.  Who doesn’t want relief from back pain or sciatica.  It can be frightening to feel that much pain and wonder if you’re going to be able to work and take care of your family.

Doctors often jump from pain relief to strengthening using physical therapy without giving much thought to joint mechanics, neuromuscular control and lastly the disc itself.  By jumping into Physical Therapy (exercises) and not working on the disc you are putting the “cart before the horse”.  You are getting things out of order and setting yourself up for disappointment and future failure.  I’ve seen it time and time again.  But there is a solution to this problem…

The disc is the most central tissue located between each of the spinal vertebrae.  It needs to be worked on first along with spinal joint alignment and nerve control from the brain to the small muscles (rotators, multifidus and intertransversarii) that rest between the bones.  These muscles act like small switches constantly giving the brain information about where joints and bones are in relationship to one another.  Without addressing this vital communication link the brain will always be slightly blind as to position of the bones and discs.  The result, loss of spinal control, chronic back/neck pain, disc degeneration and herniation and you being miserable. 

All of these structures are closest to the center where the problem is located.  Let’s call that the “CENTRAL ZONE”.  Care must start here.  Then, if you think of concentric circles moving outward our next area of focus should be addressing those “core muscles” that everyone hears about.  The core muscle are in the intermediate zone (this is where physical therapy starts).  Oops, we’ve skipped a step if we start working here, haven’t we?  Lastly, the “big mover” muscles should be strengthen.  These are your sit-up and bridging muscles called the outer zone.

Can you see the problem now?  Physical therapy starts at the core muscle zone and completely forgets about the crucial central zone where the disc, joints and jumps to the other two zones.  Is it any wonder that 80% of adults in the United States end up having at least one major back pain event in their life?  Don’t put the cart before the horse.  Getting this right means everything to your long term success.
